Medical Home in Larimer County
Health care delivered through a medical home is accessible, patient-centered, personalized, continuous, comprehensive, coordinated, compassionate and culturally responsive. A medical home team includes patients as active participants in their own health and well-being, and all of their care providers (doctors, dentists, therapists, etc.) who collaborate with each other. One of the team's professionals takes the lead in coordinating the patient's care.
In a successful medical home, every issue faced by an individual, such as medical, dental, vision, educational, psychological and developmental, are identified and addressed. The goal is to ensure individuals have access to the health care and other resources they may require.

- Patient-Centered Medical Home Providers
Miramont Family Medicine (970-482-0213), Internal Medicine Clinic of Fort Collins (970-224-9508) and the Family Medicine Center (970-495-8800) are medical practices in Larimer County officially participating in a patient-centered medical home pilot project. Other practices also may be working to become a state or nationally certified medical home.
About the Larimer County Medical Home Coalition
In summer 2009, HCC received funding from the Colorado Department of Public Health and Environment to increase understanding about the Colorado Medical Home Initiative as it pertains to the health and well being of children and youth, and develop community partnerships around the medical home approach to health care. The Larimer County Medical Home Coalition (LCMHC) was established and works to achieve these goals.
A subgroup of the LCMHC is the Healthy Harbors – Medical Home for Foster Families pilot project. This project is the beginning of building a Medical Home system in our community. It will ensure Larimer County foster children/families receive health care that is accesible, family oriented, comprehensive, continuous, coordinated, compassionate and culturally competent.
